Caseloads include outpatient services for the Independent Living, Assisted Living, and Memory Care unit in the community. PRN assistance needed for coverage during the week and/or on weekends.

Experience Requirements :

  • Minimum one (1) years clinical experience required or strong clinicals in geriatric care
  • Display strong skills in flexibility, time management, and communication
  • Solid computer skills with electronic medical record (EMR) documentation system
  • Customer Service Excellence

Position Responsibilities :

  • Physical Therapist Assistant will deliver quality physical therapy services according to treatment plans for patients
  • Document progress toward identified objectives
  • Provide accurate and timely documentation using EMR
  • Work collaboratively with the therapy team in a great team environment
  • Conduct patient and family education
  • Display strong skills in flexibility, time management, and communication
  • Superb Customer Service Excellence
