Tx:Team is seeking talented and career-minded Physical Therapist candidate for the position of Home Health Clinical Coordinator at Frederick Memorial Hospital Home Health Care in Frederick, Maryland.

As the Home Health Clinical Coordinator, you will supervise all home health therapy operations, physical therapy, occupational therapy, and speech therapy, for our partner, Frederick Memorial Hospital, under the direction of the Director of Rehabilitation Services. The right fit candidate is an effective, credible, engaging leader with the ability to motivate, influence, and hold the Home Health team accountable.

This is a Full Time position with a competitive benefit package and candidate must be a licensed Physical Therapist in the State of Maryland. Home Health Clinical Coordinator

Essential Functions:

  • Supervise operational systems in coordination with the partnering health system in the areas of clinical care, compliance, financial management including budget development, and application of reports. (QI, HHCAHPS)
  • Direct rehab therapy and specialty operations to ensure the provision of quality service consistent with applicable government laws/regulations and corporate policies / guidelines / standards.
  • Monitor the work of the Home Health clinicians to ensure efficiency, accuracy, and quality, and make provisions for their development as needed.
  • Maintain an awareness of changes in legislation that impact business operations and liability.
  • Assist in the screening and interview process of new applicants with the Home Office and ensure all associates receive training, support, and proper development to meet/exceed goals upon hire.
  • Demonstrate effective leadership skills characterized by effective communication, objective problem solving, and confidential interactions.
  • Assume accountability and ownership of operational budget of the Home Health business unit and adjusts staffing/utilization and other measures accordingly.
  • Attend designated meetings and conferences. Directs team meetings in conjunction with Director and attends regularly scheduled meetings with Director.
  • Manage supply requisitions and equipment maintenance.
  • Manage department budget with expense and revenue reporting.

Qualifications:

  • Two (2) + years Home Health clinical experience
  • Knowledge of the basic principles, literature, standard practices, methods and new developments in rehabilitation
  • Strong financial and analytical skills
  • Ability to develop and lead a thriving team culture
  • Excellent written and verbal communication skills
  • Knowledge and successful experience of the full continuum of care from inpatient to home health to outpatient
  • Successful experience working with an interdisciplinary team of clinicians, nurses, physicians, and administration
  • Customer service excellence
  • Proficient in data management, outcomes, and reporting
  • Highly organized, systematic, and able to function consistently in a fast paced environment

  • Minimum two (2) years Home Health supervisory experience
  • Degree from an accredited school approved by the American Physical Therapy Association
  • Current state licensure or eligibility in Physical Therapy
